The Toronto Western Family Health Team (TW FHT) is a team of physicians, nurse practitioners, registered nurses, social workers, pharmacists, and multiple other health care professionals who deliver excellent and comprehensive primary care.
The TW FHT is affiliated with the UHN Department of Family and Community Medicine, which trains University of Toronto medical students and medical residents, in addition to students of other health professions. You may see a student or resident during your visit with us.
