We are approximately 750m east of St. Clair Avenue West & Dufferin Street. Entrance to the clinic is on St. Clair Avenue. Garrison Creek is located on the north side of St. Clair Avenue West, east of Oakwood Avenue at Robina Avenue and St. Clair Avenue West.

Parking
There is no designated parking.   Street parking is restricted on the streets around Garrison Creek.  Please read parking signs carefully. Municipal Parking:  Green ‘P’ Parking is available.

Book an appointment with your primary care provider

For online booking with your primary care provider, you may:

• Book a 15-minute appointment, in-clinic for new concerns
• Book a 15-minute appointment, in-clinic for follow-up visits

**We are not offering online booking for appointments such physicals, well-baby visits or procedures.  If you choose to book for this type of visit, the request will be cancelled which will delay the next available appointment for you.**

• You need an appointment for an urgent issue in the next 2-3 days
• You need an appointment that is longer than 15 minutes (like physicals, procedures or a well-baby visit)
• You do not have an OHIP number
• You need an appointment with an allied health provider

Renew prescription

Please ask your pharmacy to fax a medication renewal request to Fax # 416-603-5059

• We respond to fax requests within 3 business days.
• We can not accommodate urgent prescription requests over the phone.
• If you need medication more urgently speak with your pharmacist.

Submit a photo/document
This form is only for submitting photos or documents requested by your healthcare provider. We will accept documents in these formats (JPEG, PNG, PDF, text and standard media files) and they can be a maximum of 10MB. If the files are larger than 10MB, please send in multiple forms.

We will inform your provider that you have made a submission and place into your chart.
