We are a health care team who come to your home when you are no longer able to visit the doctor's office.
We will support you to:
- live safely in your home
- find community resources
- reduce emergency room and hospital admissions
- make future plans
Home-Based Care Program is part of of Family Health Team at Toronto Western Hospital, University Health Network.

We value your feedback and will partner with you to provide the best care.
Answer these questions to see if you can be part of the program. Are you:
- 65 years or older?
- Living within the postal codes M5V, M6G, M6H, M6J, or M6K?
- Having a major difficulty getting to your family doctor?
- Willing to transfer care from your current family doctor to the Home-Based Care Program?
If you answered 'yes' to the questions above, you may qualify for the Home-Based Care Program.
Once you become our patient
- Call 416-603-5380 to make appointments or for consultations
- Please say you are a Home-Based Care patient when you call

Monday to Friday - 9:00 am to 5:00 pm

Important:
- Call 911 in case of an emergency
- Home-Based Care Program is NOT for emergency visits
