Family Health Teams are interdisciplinary team based primary health care organizations that provide comprehensive family health care services for their community. 

The Toronto Western Family Health Team (TW FHT) was established in 2008. TW FHT is built on a long-standing family medicine department at Toronto Western Hospital.  The TW FHT is governed by its Board of Directors , and is partnered with the University Health Network Department of Family and Community Medicine , an academic teaching unit fully affiliated with the University of Toronto.
The TW FHT: 

  • Offers a wide range of programs and services: including health promotion services, chronic disease management programs, home based care program, and obstetrical care services.
  • Is highly engaged in the training of medical students, residents and other health professional trainees and active in a range of research initiatives.
  • Has partnerships with over 20 community organizations to collaborate in the planning and delivery of services to a wide range of programs and population groups
  • Has an extensive focus on patient safety and quality improvement

  • Our interdisciplinary team consists of:  family physicians, nurse practitioners, registered nurses, registered practical nurses, social workers, dieticians, chiropodists, occupational therapist, physiotherapist, respiratory therapist, pharmacist and a health education specialist
  • Catchment area: St. Clair Avenue West to Lake Ontario; Dufferin Street to Yonge Street
  • Roster size of approximately 18,000 enrolled patients
  • Residency Program: Our health care providers are also teachers at the University of Toronto.  Resident doctors, senior medical students and other health care students will help with your care while being supervised by fully qualified staff.
